IT Staff Awards Nomination Form

The Information Technology Staff Awards recognizes individual staff members who have been selected as providing outstanding service or support, or have made significant contributions to the department and university community.

To be nominated, employees should meet certain eligibility requirements:

  • Any staff employee (regular or contract) is eligible to be nominated after one year of continuous service.
  • Nominees must be an active employee at the time of the award.
  • Nominees must meet the one year time frame at the time nominations are accepted and must be a current employee in the Office of Information Technology.
  • Awards are for individuals and not teams within the Office of Information Technology
  • All nominations must be made by faculty, staff or students.
  • Self-nominations will not be considered.
  • Nominations must be submitted on-line.

Please describe how the individual you are nominating has actively supported the IT Vision and Mission during the preceding year.

Examples include supporting the department or university by:
  • Improving of a business process or service process.
  • Offering innovative ideas or researching new technology.
  • Exceeding customer expectations through service or support.
  • Providing exceptional training.
  • Mentoring of others.
  • Sharing of knowledge.
  • Fostering teamwork and collaboration.
